REPORT OF RAILROAD SAFETY VIOLATIONS/INCIDENTS AND REMOTE CONTROL ACCIDENT/INCIDENT FORM
If there have been any accidents, with or without injuries, involving Remote Control Operations, please complete the second half of the safety form. You must fill in the contact information at the bottom of the form for it to be accepted. All information will be kept strictly confidential. Please fill out the form and either print it and return to us at the address on the bottom of the form. We will then forward it to the International Division. You may also send it to us by fax or click the 'E-Mail' button at the top of the form to send it electronically. If you have any pictures, send copies of them to us for handling and posting to our website.
